8D and defect handling: how a charger factory should answer a complaint

A complaint answered with a discount is a complaint answered twice. The 8D structure exists because the intermediate steps are what stop the fault recurring.

Field complaints arrive in the same shape almost every time: a unit failed, the customer wants to know why, and they want to know what stops it happening again. The structure that answers both is 8D — eight disciplines, of which four carry the value.

The steps that matter

Containment (D3). What stops the same fault from reaching other customers today — a hold on stock, a check on the next shipment, a re-test of units in transit. Containment should happen before the root cause is known.

Root cause (D4). Not “the component failed” but why it failed, and why the process allowed it through. A good answer names both an occurrence cause (what made the fault) and a detection cause (why inspection missed it).

Corrective action (D5). The change, and where it was applied: component, process, tooling, inspection plan. Vague answers like “retraining” usually mean the cause was not found.

Verification (D6–D7). Proof that the fix works — the re-tested batch, the updated inspection record, the date from which the change applies.

What buyers should ask for

Three dates: when containment took effect, when the corrective action starts applying to new production, and when the verification batch results are available. Then ask which earlier shipments are affected. A factory that can answer that last question without searching is one that has traceability across batches.

What it costs to skip

A complaint handled only commercially comes back — sometimes from another customer, sometimes from a different market, and often in larger volume. The engineering time in 8D is cheap next to the cost of finding the same fault again at container scale.
